Diagnóstico y manejo de enfermedad diverticular complicada - Revisión narrativa

La enfermedad diverticular complicada representa un desafío clínico significativo debido a su diversidad de presentaciones y posibles complicaciones graves, como abscesos, perforaciones, fístulas y obstrucción intestinal. Este artículo de revisión narrativa tiene como objetivo proporcionar una visión integral sobre el diagnóstico y manejo de esta patología, basada en la evidencia más reciente. El diagnóstico temprano y preciso es fundamental para orientar el tratamiento adecuado. Las herramientas de imagen, como la tomografía computarizada, son esenciales para evaluar la extensión de la enfermedad y clasificar su gravedad. En cuanto al manejo, este varía desde un enfoque conservador con antibióticos y drenaje percutáneo en casos seleccionados, hasta intervenciones quirúrgicas en situaciones más complejas o refractarias. La elección del tratamiento debe individualizarse, considerando factores como la condición clínica del paciente, comorbilidades y recurrencia de episodios. Además, se discuten estrategias para la prevención de recurrencias, incluyendo cambios en el estilo de vida y, en algunos casos, cirugía electiva. Este artículo resalta la importancia de un enfoque multidisciplinario para optimizar los resultados clínicos y mejorar la calidad de vida de los pacientes afectados por esta condición.
