Diagnóstico y manejo de la enfermedad inflamatoria intestinal - Revisión narrativa

La enfermedad inflamatoria intestinal (EII), que incluye la enfermedad de Crohn y la colitis ulcerosa, es un grupo de trastornos crónicos caracterizados por inflamación del tracto gastrointestinal. Su etiología es multifactorial, involucrando factores genéticos, inmunológicos y ambientales. El diagnóstico de la EII se basa en una combinación de criterios clínicos, endoscópicos, histológicos y de imagen, siendo fundamental un enfoque multidisciplinario para su identificación precisa. Los avances en biomarcadores, como la calprotectina fecal, han mejorado la detección y el monitoreo de la actividad de la enfermedad. En cuanto al manejo, este se enfoca en inducir y mantener la remisión, mejorar la calidad de vida y prevenir complicaciones. Las opciones terapéuticas incluyen tratamiento farmacológico, como aminosalicilatos, corticosteroides, inmunomoduladores y agentes biológicos, así como intervenciones quirúrgicas en casos refractarios o con complicaciones. Además, el papel de la nutrición y el soporte psicológico es cada vez más reconocido en el tratamiento integral del paciente. Este artículo revisa los aspectos clave del diagnóstico y manejo de la EII, destacando los avances recientes y las perspectivas futuras en el abordaje de esta compleja patología.
