Aplicación de imagenología intraoperatoria con fluorescencia en la evaluación de márgenes quirúrgicos en tumores abdominales

La utilización de la imagenología intraoperatoria con fluorescencia ha emergido como una herramienta innovadora en la cirugía oncológica, especialmente en el manejo de tumores abdominales. Este enfoque permite una evaluación en tiempo real de los márgenes quirúrgicos, aspecto crucial para garantizar la resección completa del tumor y minimizar la recurrencia local. La técnica se basa en el uso de agentes fluorescentes que se acumulan selectivamente en el tejido tumoral, proporcionando una diferenciación visual clara entre tejido sano y maligno durante el procedimiento quirúrgico. Diversos estudios han demostrado que esta tecnología mejora significativamente la precisión de los cirujanos al identificar márgenes comprometidos, reduciendo la necesidad de reintervenciones y mejorando los desenlaces oncológicos. Además, su integración en la práctica clínica ofrece un enfoque menos invasivo en comparación con métodos tradicionales como el análisis histopatológico intraoperatorio. Sin embargo, existen desafíos asociados, como la variabilidad en la especificidad de los agentes fluorescentes y su costo, que limitan su implementación generalizada. Este artículo revisa los avances recientes, las aplicaciones clínicas y las limitaciones actuales de esta técnica, destacando su potencial para transformar el estándar de cuidado en la cirugía oncológica abdominal.
