Enfermedades glomerulares: enfoques actuales en diagnóstico y tratamiento

Las enfermedades glomerulares representan un grupo heterogéneo de patologías que afectan la estructura y función de los glomérulos renales, desempeñando un papel clave en la fisiopatología de la enfermedad renal crónica. Este artículo de revisión narrativa aborda los avances contemporáneos en el diagnóstico y tratamiento de estas condiciones, destacando la importancia de un enfoque multidisciplinario. En términos diagnósticos, la integración de herramientas tradicionales como la biopsia renal, con técnicas avanzadas como la microscopía electrónica y los estudios genéticos, ha permitido una caracterización más precisa de las diferentes entidades glomerulares. Por otro lado, los tratamientos han evolucionado significativamente, incorporando terapias dirigidas que modulan procesos inmunológicos específicos, como el uso de inhibidores del complemento y anticuerpos monoclonales. Asimismo, se enfatiza la necesidad de individualizar el manejo según las características clínicas, histológicas y moleculares de cada paciente. Este enfoque personalizado no solo mejora los resultados clínicos, sino que también minimiza los efectos adversos asociados a tratamientos inespecíficos. Finalmente, se discuten los retos actuales en la práctica clínica, incluyendo el acceso limitado a recursos diagnósticos avanzados en ciertos contextos y la necesidad de investigaciones adicionales para optimizar las estrategias terapéuticas. Este artículo busca proporcionar una visión integral y actualizada para los profesionales involucrados en el cuidado de pacientes con enfermedades glomerulares.
