Retos y avances en el manejo de la retinopatía diabética - revisión narrativa

La retinopatía diabética (RD) representa una de las principales complicaciones microvasculares de la diabetes mellitus y constituye una causa importante de ceguera en el mundo. Este artículo de revisión narrativa aborda los retos y avances recientes en su manejo, destacando la importancia de un diagnóstico temprano y un tratamiento oportuno para prevenir la progresión de la enfermedad. Entre los desafíos más relevantes se encuentran el manejo integral de los factores de riesgo, como el control glicémico, la hipertensión y la dislipidemia, así como el acceso limitado a servicios especializados en ciertas regiones. En cuanto a los avances, se han desarrollado tecnologías innovadoras como la imagenología avanzada y la inteligencia artificial para mejorar la detección y el monitoreo de la RD. Además, las terapias intravítreas con agentes antiangiogénicos han demostrado ser altamente efectivas en el tratamiento del edema macular diabético y en la prevención de complicaciones severas. Sin embargo, persisten barreras relacionadas con la adherencia al tratamiento y la equidad en el acceso a estas terapias. Este panorama resalta la necesidad de estrategias multidisciplinarias que combinen prevención, educación y avances tecnológicos para optimizar el manejo de la retinopatía diabética y mejorar los resultados visuales a largo plazo.
