Avances recientes en el tratamiento de cicatrices hipertróficas y queloides: enfoque dermatológico y quirúrgico

Las cicatrices hipertróficas y los queloides representan un desafío significativo en dermatología debido a su impacto funcional y estético. Este artículo de revisión narrativa aborda los avances recientes en el manejo de estas lesiones, considerando tanto enfoques dermatológicos como quirúrgicos. En el ámbito dermatológico, se destacan terapias emergentes como la administración tópica y sistémica de moduladores de cicatrización, el uso de láseres fraccionados y no ablativos, y la aplicación de corticosteroides intralesionales, que han demostrado eficacia en la reducción del volumen cicatricial y la mejora de los síntomas asociados. Por otro lado, las técnicas quirúrgicas han evolucionado hacia procedimientos más conservadores y combinados, como la escisión seguida de radioterapia superficial o crioterapia, para minimizar el riesgo de recurrencia. Además, se exploran terapias adyuvantes innovadoras como los inhibidores de citocinas y factores de crecimiento, que actúan sobre las vías moleculares implicadas en la fibrosis. A pesar de estos avances, persisten desafíos relacionados con la heterogeneidad en las respuestas al tratamiento y la necesidad de enfoques personalizados. Esta revisión resalta la importancia de una estrategia multidisciplinaria para optimizar los resultados clínicos y mejorar la calidad de vida de los pacientes afectados por estas condiciones.
