Abordaje integral de las enfermedades hepáticas crónicas: de la prevención al trasplante

El abordaje integral de las enfermedades hepáticas crónicas es un desafío prioritario en la práctica clínica debido a su alta prevalencia y las complicaciones asociadas. Este artículo de revisión narrativa aborda desde las estrategias de prevención, incluyendo la promoción de estilos de vida saludables y la vacunación, hasta los avances en el diagnóstico temprano mediante técnicas no invasivas como la elastografía. Asimismo, se destacan las opciones terapéuticas actuales, que abarcan desde el manejo médico de las comorbilidades hasta el tratamiento específico según la etiología, como antivirales para la hepatitis viral o agentes antifibróticos en desarrollo. También se discute el papel crucial del seguimiento multidisciplinario para prevenir la progresión a estadios avanzados, como la cirrosis o el carcinoma hepatocelular. Finalmente, se analiza el trasplante hepático como tratamiento definitivo en casos seleccionados, subrayando la importancia de la selección adecuada de candidatos y el manejo postrasplante para optimizar los resultados. Este enfoque integral, basado en la prevención, el diagnóstico precoz y el tratamiento oportuno, es fundamental para mejorar el pronóstico y la calidad de vida de los pacientes con enfermedades hepáticas crónicas, reduciendo así su impacto en los sistemas de salud.
