Nuevos enfoques en el tratamiento del glaucoma: dispositivos y farmacología

El glaucoma es una enfermedad neurodegenerativa crónica que constituye una de las principales causas de ceguera irreversible a nivel mundial. A pesar de los avances en su diagnóstico y manejo, el control adecuado de la presión intraocular (PIO) sigue siendo el pilar fundamental del tratamiento. En los últimos años, han surgido nuevos enfoques terapéuticos que incluyen tanto innovaciones farmacológicas como el desarrollo de dispositivos médicos diseñados para mejorar la eficacia, seguridad y adherencia al tratamiento. Entre los avances farmacológicos destacan las nuevas formulaciones de agentes hipotensores oculares, así como compuestos dirigidos a dianas moleculares específicas que buscan frenar la progresión del daño glaucomatoso. Por otro lado, los dispositivos microinvasivos para el manejo del glaucoma (MIGS, por sus siglas en inglés) han revolucionado el abordaje quirúrgico, ofreciendo alternativas menos invasivas y con menores riesgos en comparación con las técnicas tradicionales. Este artículo revisa de manera narrativa los avances más recientes en el tratamiento del glaucoma, analizando su impacto clínico, las evidencias disponibles y las posibles limitaciones, con el objetivo de proporcionar una visión integral que facilite la toma de decisiones en la práctica clínica y fomente un manejo más personalizado de esta patología compleja.
